We accept a limited number of insurance plans.

Since every insurance policy on coverage is unique and can vary greatly in their reimbursement terms, it is YOUR responsibility to check with your insurance company BEFORE your visit, to be sure that these services are covered.

As the policyholder, you will be held responsible for your bill, should your insurance company determine that the services are not covered.

Depending on the insurance plan, a doctor’s referral for your visit may be necessary.

In order to bill your insurance, we require the following:

• A current insurance card of your primary insurance and if applicable, of your secondary insurance carrier.
• Primary policyholder’s name and date of birth.
• Co-payments are paid at the time of service.

The evaluation and treatment sessions with the Schroth Method are billed under standard physical therapy codes.

Regarding insurances that we don’t carry, but that will reimburse you retroactively for out-of-network physical therapy services:

• Call your insurance and verify their terms of reimbursement prior to your first session.
• You will be responsible for payment after each session.
• We will provide you with a receipt of payment and all other documentation that is required by your insurance company.

We do not accept credit cards.

Private Pay
For private pay, please inquire about the current fee schedule. Fees are payable after each session.

For no shows or same day appointment cancellations, we reserve the right to charge a fee of $ 40.
