• Download, print and fill out this attached on-line questionnaire and bring it with you to your first appointment.
• Mail x-ray, if available, prior to your appointment.
• Bring relevant doctor’s reports to your first visit, if available.

More specific questions regarding concerns about your scoliosis can only be answered after you have been evaluated.

Wear comfortable clothing and an appropriate top for proper evaluation of your spine and posture.
